Adult Passport Contest
Collect 15 cap stickers, earn rewards, and submit your completed passport for the Adult Grand Prize drawing.
- ⚽ Adult playoff cap stickers count as bonus entries only.
- 🏆 One passport per person.
- 📸 Upload front and back to enter.
Kids Passport Contest
Collect 6 stickers, earn kid-friendly rewards, and submit the completed Kids Passport for the Grand Prize drawing.
- 🍦 Free ice cream, soccer prize, kids meal, and Grand Prize entry.
- 🎉 Grand Prize: pizza party for up to 8 kids.
- 📸 Parent or guardian should submit the form.

Submit Your Adult Passport
For guests 18+. Upload clear photos or scans of the front and back of your completed Adult Game Passport.
Submit Your Kids Passport
For guests 17 & under. Parent or guardian contact information should be used for all Kids Contest submissions.
★ Official Rules ★
- One cap/sticker per guest per visit.
- Order any food item and receive ONE sticker.
- Staff must initial rewards to be valid.
- Upload your completed passport to enter.
- Adult playoff cap stickers are bonus entries only — they do not count toward the 15 reward cap stickers.
- Kids playoff stickers count toward your 6 stickers and give 1 bonus Grand Prize entry each.
- One passport per person. All entries become property of The ’401 Table and Tap.
